How to Read eGFR Results Without Guesswork.
Learn how to read eGFR results, what the ranges may mean, and when kidney function changes require prompt nephrology review and follow-up care planning.
A laboratory report may show an eGFR of 48, 82, or 16 with little explanation beside it. Knowing how to read eGFR results can help you ask the right questions, but the number should never be interpreted in isolation. A nephrologist considers the trend over time, urine protein, symptoms, blood pressure, medications, and the clinical reason the test was ordered.
What eGFR Measures
Estimated glomerular filtration rate, or eGFR, is an estimate of how effectively the kidneys filter waste products from the blood. It is generally calculated from your serum creatinine level, along with age and sex. Many laboratories in the United States now use the 2021 CKD-EPI equation, which does not include race as a calculation factor.
Creatinine is a waste product generated by muscle activity and cleared mainly through the kidneys. When kidney filtration falls, creatinine commonly rises and the eGFR falls. Because the calculation is an estimate rather than a direct measurement, it provides a useful clinical signal, not a complete diagnosis.
Most reports list eGFR in milliliters per minute per 1.73 square meters of body surface area, written as mL/min/1.73 m². This standardized format allows clinicians to compare results across patients and laboratories.
How to Read eGFR Results by Range
For adults, an eGFR of 90 or higher is generally considered within the expected range. However, a person can still have chronic kidney disease if there is evidence of kidney damage, such as albumin in the urine, persistent blood in the urine, structural abnormalities on imaging, or a history of kidney transplantation.
An eGFR from 60 to 89 may be mildly reduced. In older adults, this may occur without progressive kidney disease. In younger adults, or in anyone with diabetes, hypertension, abnormal urine findings, or a falling eGFR trend, it deserves closer assessment.
An eGFR below 60 that persists for at least three months may meet the definition of chronic kidney disease. The standard GFR categories are:
- G1: 90 or higher, with evidence of kidney damage
- G2: 60 to 89, with evidence of kidney damage
- G3a: 45 to 59
- G3b: 30 to 44
- G4: 15 to 29
- G5: below 15
These categories describe filtration level, not the full severity of a patient's illness. A patient with an eGFR of 55 and substantial urine protein may face a greater risk of progression and cardiovascular complications than someone with a stable eGFR of 40 and no albuminuria. The underlying cause of kidney disease also matters greatly.
One Result Is Not a Kidney Disease Diagnosis
A single low eGFR does not automatically mean permanent kidney damage. Temporary changes can occur with dehydration, vomiting, diarrhea, fever, severe infection, heart failure, urinary obstruction, or use of certain medications. In Jamaica's heat, fluid loss during illness or outdoor work can be particularly relevant, especially for older adults and people taking diuretics.
Clinicians distinguish chronic kidney disease from acute kidney injury by reviewing prior laboratory values and the timing of the change. Chronic kidney disease requires an abnormality that is present for at least three months. Acute kidney injury can develop over hours to days and may require urgent evaluation, particularly when urine output drops or creatinine rises quickly.
For this reason, compare your current eGFR with earlier results whenever possible. A stable eGFR of 35 over several years requires careful long-term nephrology care. A decline from 75 to 35 over a short period requires a different level of urgency and investigation.
The Urine Albumin Test Changes the Interpretation
An eGFR result is only one part of a kidney assessment. The urine albumin-to-creatinine ratio, often called UACR or ACR, measures whether albumin is leaking into the urine. Healthy kidneys usually prevent significant albumin loss.
A UACR below 30 mg/g is generally considered normal to mildly increased. A result of 30 to 300 mg/g indicates moderately increased albuminuria, while a result above 300 mg/g indicates severely increased albuminuria. Higher albumin levels are associated with greater risk of kidney disease progression and cardiovascular disease.
This is why a person with an eGFR of 95 may still need nephrology evaluation if they have persistent high urine albumin, especially with diabetes or uncontrolled blood pressure. Conversely, an eGFR that is mildly reduced without urine abnormalities may have a different prognosis. Kidney care should be based on both filtration and urine findings, not either test alone.
Why eGFR Can Be Less Accurate in Some People
Because eGFR is calculated from creatinine, anything that changes creatinine production can affect the estimate. People with unusually high muscle mass may have a higher creatinine despite normal filtration. Frail older adults, individuals with low muscle mass, and people with significant weight loss may have a deceptively low creatinine, which can make eGFR appear better than true kidney function.
A large cooked-meat meal shortly before testing, creatine supplements, intense exercise, pregnancy, severe liver disease, and certain medications may also affect interpretation. Trimethoprim, for example, can raise creatinine without necessarily causing a true fall in filtration. Anti-inflammatory medicines such as ibuprofen or naproxen can, in some circumstances, reduce kidney blood flow and contribute to genuine kidney injury.
When creatinine-based eGFR may be unreliable, a clinician may request cystatin C testing or a combined creatinine-cystatin C eGFR. This is often useful when the diagnosis is uncertain, particularly in people with an eGFR of 45 to 59 and no other clear marker of kidney damage.
What a Falling eGFR Should Prompt
A lower eGFR should lead to a structured clinical review. That review commonly includes blood pressure control, diabetes management, medication reconciliation, urine testing, electrolyte levels, anemia screening, and renal imaging when obstruction or structural disease is suspected.
Do not stop prescribed blood pressure medicines, diabetes medicines, or diuretics solely because you see a lower eGFR on a report. Some medicines, including ACE inhibitors, ARBs, and certain SGLT2 inhibitors, can cause small early changes in laboratory values while providing important long-term kidney and cardiovascular protection. Their use requires individualized monitoring rather than self-directed discontinuation.
As eGFR declines below 30, complications become more common, including anemia, high potassium, metabolic acidosis, mineral and bone disorders, fluid retention, and medication accumulation. This is a stage when specialist nephrology oversight is particularly valuable. Planning for dialysis or transplantation, when appropriate, should begin well before kidney failure becomes an emergency.
An eGFR below 15 does not automatically mean dialysis must start immediately. Dialysis decisions are based on symptoms, fluid overload, uncontrolled potassium, severe acidosis, nutritional decline, and overall clinical condition, not a number alone. Planned care is safer than emergency dialysis initiation.
When to Seek Prompt Medical Attention
Contact a clinician promptly if a low or rapidly changing eGFR occurs with markedly reduced urine output, swelling, shortness of breath, persistent vomiting, confusion, chest symptoms, severe weakness, or very high blood pressure. These features may signal complications requiring same-day assessment or emergency care.
Patients receiving dialysis, those with a kidney transplant, and travelers who depend on scheduled dialysis should treat missed treatments, access problems, fever, or breathing difficulty as urgent issues. Continuity of laboratory records, dialysis prescriptions, and medication lists can materially improve the safety of care when treatment is coordinated across locations.
An eGFR result is most useful when it starts a disciplined conversation: What was my previous value, is there albumin in my urine, what is causing the change, and what should happen next? Those answers turn a laboratory estimate into a practical plan for protecting kidney function and avoiding preventable emergencies.
